Appointments/First Session Information

 

To make an appointment please call our office at (678) 263-8512. If you are a physician’s office please fax insurance and medical information to 678-298-9997. We are open Monday through Thursday from 10:00 a.m. until 5:00 p.m, and every other Friday. Please note if leave a message on a Friday we are closed, we will return the message the following Monday.



First Session Information and Initial Paperwork

Because assessment involves a professional relationship, we need to agree on the terms of our work together. This is explained in detail in the in the initial paperwork which will be given to you at your first appointment. The paperwork represents your understanding of our agreement with our terms of service. If you have any questions and/or require clarification, please be sure to ask us.
